Could This Be SIBO Methane?
I am a 22 year old male having digestive issues for about 5 months. My symptoms include constipation with sticky stools, a feeling of incomplete bowel movement, foul smelling gas, and bloating soon after eating when constipated. I consulted a gastroenterologist who prescribed Linaclotide and Movicol and advised dietary changes and exercise, which I followed, but I have not experienced significant relief, when I discussed with him about SIBO he refused to get me tested. About a two months ago I took ofloxacin + ornidazole for 5 days and noticed slight improvement, but the symptoms returned. I am wondering if this could be related to SIBO, especially methane related, and whether a breath test would be appropriate. Would treatment options like rifaximin with metronidazole be considered in such cases?

Surgery to fix lax cardia (Lose LES)
I was recently diagnosed with Lax Cardia (incompetent Lower Esophageal Sphincter) via endoscopy. My primary symptoms are constant throat irritation and regurgitation. Despite strictly following medical therapy (PPIs), diet modifications, and lifestyle changes, I have seen no significant improvement.
I am now considering surgical or endoscopic interventions. Specifically, I would like to know:
Are the LINX Reflux Management System or the Stretta Procedure currently available at any hospitals in Bangalore?
What is the approximate cost for these procedures in the city?
Based on my symptoms, would you recommend one over the other, or should I consider a traditional Nissen Fundoplication?

Intramocusal andenocarcinoma on rectum
Hello Doctor.
I had intramucosal adenocarcinoma of the rectum about 9 years ago, arising from a 2 × 2 cm tubulovillous adenoma, moderately differentiated, with no invasion beyond the muscularis mucosa. Only polypectomy was performed, and no further treatment was required. The report showed MSI-negative / MMR-proficient.
At that time, MRI of the pelvis and a CT scan were normal, and the post-polypectomy site biopsy showed no dysplasia or malignancy.
Follow-up has been done regularly: • Colonoscopy – about 10 times, all normal (last on 12 November 2025)
• Whole abdomen ultrasound – about 10 times, all normal
• Blood tests – about 6 times, all normal
• Chest X-ray (PA view) – 4 times, all normal
Kindly advise the appropriate interval for my future surveillance colonoscopy. Also, if a new polyp appears in the future, would it necessarily be malignant in my case, or would it usually be benign? If I undergo colonoscopy every year, what is the risk that a new polyp could be malignancy?
