Gallstones, Suggestions & Next Steps
My wife, 31F, has multiple gallstones (3–6 mm) with sludge, but no cholecystitis and normal CBD on USG. Grade 1 Fatty Liver. She has recurrent episodes of upper abdominal pain with back/right-sided back pain, nausea, and the latest episode lasted 2–3 hours with vomiting, after which she felt better. Recent LFT: AST 88, ALT 114, direct bilirubin 0.88, with normal ALP, amylase and lipase. Gastroenterologist diagnosed dyspepsia + cholelithiasis and has advised surgical opinion for cholecystectomy. 1. Do these symptoms sound like symptomatic gallstones/biliary colic? 2. How urgent is laparoscopic cholecystectomy in this situation? Can we safely wait and observe, or should we plan surgery soon? 3. Should any further tests (repeat LFT/MRCP/endoscopy) be done before deciding on surgery?
