Treatment Resistant Depression

Mr Patra, a 64-year-old retired Coal India employee, presented with persistent low mood, no interest in daily activities, talking to people, or physical activity, plus reduced appetite, weight loss, and insomnia. He also has a hiatal hernia and GERD. In 2016, he had repetitive thoughts and sleeplessness, managed for four years with Fluoxetine 20 mg, Clomipramine 25 mg, and Lorazepam 2 mg. In 2020, an ischemic stroke (PCA territory) triggered a relapse of depression. Prior meds failed; Fluoxetine rose to 80 mg (no effect), Escitalopram 20 mg (ineffective), Venlafaxine 75 mg with Bupropion 300 mg (helped but stopped due to gastritis), and Mirtazapine (unhelpful). Lorazepam dependence persists despite trials of Clonazepam, Diazepam, Melatonin, and Trazodone. For four years, refractory GERD, on Esomeprazole and Levosulpiride, continues. Currently, he has ongoing low mood, disinterest in socializing and activity, poor appetite, weight loss, and insomnia, with treatment-resistant depression.

Brain fogging

Hii I have suffering a problem in which after I woke up I don't remember much, I'm forgetting everything, EVEN I don't remember the names of my office colleagues, and my mind is like it's out of my control I have a headache everytime, feeling heavyness on my head, I don't know why, but I start feeling active after 7pm . The problem just vanished after 7 pm I don't know what is happening Yesterday I took full body checkup It shows severe deficiency of vitamin D & B12 except these all the reports are normal This is happening to me since 3 years Please advise me

Do psychiatric meds that affect neurotransmitter and receptors in the brain, cause any death of brain cells/ kill neurons? Do they reduced intelligence? And, only if receptors are affected, can receptors be rebuilt over time? In short, do psychiatric medicines kill brain cells? Please guide corry. Thanks

Hallucinations

Hello respected mam/sir, I am Nishka Singh, an MBBS student first year. I am away from my family and I live in hostel. When I was preparing for NEET I had depression and I didn't had much friends. I had a male best friend (now also he is here in my life but he is also in another college). I use to cry a lot for no reason (most of the episodes occurred in night). I shared my problem with my friend and he understood me and after sometime we started talking a lot and shring our feelings. I felt more connected with him and fell in love with him. I confessed my feelings because I thought it would be good. Nothing much happened between us, he explained me that we cannot be in a relationship and I understood. After that my episodes increased but somehow I managed. Now while I am alone in completely other state I imagine him with me and I talk to him in my imagination almost whole day since 3 months. I used to imagine someone else while preparing for NEET because I had no friends and talk.

My dad age is 70 Since last 15 days , he is facing overthinking and more observation related issues He is taking pictures of a surroundings Recently He said to me, so he is overthinking thinking if he is looking at something is observing it more, what it is, what that can be kind of thing and taking that picture So to control himself, he is not looking anything. Just he is closing guys, he is controlling himself He is fear of looking at something if he see something he’s thinking about that too much and he’s taking photographs of the objects For example, just now, we visited one hospital, which is going so many years and familiar to him, even though he took the main gate of the hospital And he’s taking pictures of me and his daughter.

No sleep even after lorazepam

Male 26 It's been 2 months since my anxiety meda stopped. (escitopram 5mg). No anxiety since. I have recurrent sleep problem though. I had some lorazepam left over (since it was sos) which I occasionally use 3-4 times a month (not week but month). But I have noticed even after lorazepam 2mg I am not able to sleep. Please suggest. (no anxiety or depression I can detect) Could it be because I might have exams around and even though I'm not anxious but somewhere deep down it's affecting.
