Am having irregular bowel movements; & getting extremely hard stool, for the past 3-4 weeks. What should I do?

I have been having difficulty swallowing for more than a month now. I feel scared to swallow food or medicines because i feel food will be stuck in the throat or will go inside the wind pipe. Also recently for that i am eating slowly and i have a low appetite as well. Attaching few reports. Please suggest

I think I am suffering from a prolonged digestion problem, feels bloated, gas and stool looks like odd smelling and slightly loose, stomach cramps frequently, all this happening since many Few years, can someone help or suggest, i change my diet and nothing happens, i drink a lot of water, eat oats in breakfast but no big improvement..

Pain occurs in the middle of  abdomen/stomach late night and then gone in the morning. Frequency of pain is 6 times from the last 30 days Do I have to worry ?

6 months ago I had right upper quadrant cramp pain Colonoscopy and Endoscopy was Normal BILLIRUBIN - 0.82 ( Direct 0.51 + Indirect 0.31) SGOT 36 SGPT 45 USG whole abdomen normal - did that 2 years ago Now yesterday after eating and drinking (2drinks) again cramp like pain in same area.
