Hello Dr I have epigastric area cramps bloating pain and altered bowel movements 5 to 7 times with burning in stool mucus everyday from 17 years. And from 10 years I have slow urine flow. Taken many medicines for 2 years from 2 to 4 psychiatrists normaxin sertraline mirtaz clonazepam duloxetine paroxetine amytriptyline colospa x rabesec ls more than 10 psychiatric medicine but no relief in stomach problems and urine problem. I have also social anxiety from 25 years also. I want to know that Is really IBS curable by psychiatrist medicine and psychotherapy & cbt etc. Please help any psychiatrist who had cured cases of these health issues.

Persistent palpitations (fast heartbeat) Poor appetite, difficulty gaining weight Ongoing weight loss Sleep disturbance (both onset and maintenance insomnia) Constipation and gastric discomfort Anxiety and restlessness during the day Current prescription: Sertraline 150 mg Olanzapine (night) Clonazepam 0.25 mg (as needed) Concern / Question: It has been more than 35 days on this regimen, but my appetite, sleep, weight, and palpitations have not improved. At lower doses of Sertraline (50–100 mg), I felt partial relief with fewer side effects. Should the Sertraline dose be reduced, or should an alternative/add-on (such as Mirtazapine or Prothiaden) be considered to improve appetite, sleep, and weight along with controlling palpitations?

Doctors I have anxiety since 5 years . anxiety of dying with heart attack.i fell I get heart attacks and I will die.thats anxiety I will accept now..but nowadays whenever I get nap of sleep or some sleep in afternoon on my shop ..I can't describe what I fell ..little nap gives me anxiety I can't this happen to me when I sleep I get anxiety other wise okk ...

HISTORY & CASE SUMMARY :- This 64 years old male presented with C/o altered behavior since 1 day. The patient is k/c/o - Hypertension on regular medication. The patient was admitted and routine investigation done, serum sodium -119, serum potassium - 4.0, CBC-Hb- 14.1, WBC- 13000, platelet -3.49 lacs, HBA1C- 6.3%, Total IgE-46.16, LFT - Total Bill - 0.58, SGOT-32.6, SGPT- 16.8, RFT, urea-13.0, Creatinine - 0.76, the patient was started on 3% nacl infusion and Nat-level corrected to 127. The patient's sensorium improved and was stabilized, urine R/M- normal, USG abdomen fatty hepatomegaly (Grade-I), BPH - (Grade-I). a psychiatrist opinion was taken I/V/O - acute psychosis and advise followed. The next day serum sodium -133.0. the patient was given BZD, NS (IV fluids), antibiotics, anti-psychotic, analgesic, PPI, anti-hypertensive, antiepileptic, the patient improved gradually and is being discharged in stable condition, But in home , he continues senseless speaking
