Need Your Advice on the Next Step

Hi Dr. I'm really worried. I recently saw a local psychiatrist for recurrent panic attacks & was prescribed Clonazepam 0.25 mg SOS. Today @ clg I had one of the worst panic attacks I've had, even though there was no obvious trigger. I took 1 tab exactly as advised, but it didn't seem to help. The attack lasted around 20–30 mins. I've been getting these attacks even w/o stress. I've already had cardio tests (ECG/Echo, etc.), and they were normal. I'm not sure what to do now. Should I continue with the same psychiatrist or consult you? I can visit offline in Hyderabad if needed. Pls advise.

Adult adhd level 1

Hello Doctors, I am an adult seeking a comprehensive online assessment for possible Autism Spectrum Disorder (Level 1) and/or ADHD. I have experienced lifelong difficulties with social communication, sensory sensitivities, executive functioning, attention, memory, and daily functioning. These traits have been present since childhood but were never formally evaluated. Unfortunately, when I have tried discussing this before, some doctors dismissed my concerns simply because I am an adult, saying I am "too old" to have autism or ADHD. From what I understand, many adults are diagnosed later in life, especially if they learned to mask their symptoms. I am looking for a psychiatrist or clinical psychologist who has experience with adult autism and ADHD assessments and is open to conducting a thorough evaluation online using appropriate diagnostic methods, rather than ruling it out based on age alone. If you provide such assessments or can guide me, I would really appreciate your response

What's your clinical experience?

I'm looking for input from psychiatrists. I don't want to use stimulant medications for ADHD. I started atomoxetine, but after a few days I developed urinary retention, so my doctor stopped the medication. My question is: is it reasonable to consider adding tamsulosin and retrying atomoxetine under medical supervision? Is this an approach that's ever used in clinical practice? Does tamsulosin usually help with atomoxetine-induced urinary retention? Would tamsulosin be expected to reduce atomoxetine's effectiveness for ADHD? Are there any important risks or reasons this combination should generally be avoided? I'm not looking for personal medical advice—just interested in how psychiatrists think about this situation in practice.

Sexual Dysfunction – Need Psychiatric Op

Hello Doctor, I have had persistent sexual function problems for the past 6 years, including difficulty getting and maintaining erections, reduced morning erections (about once a week), and reduced spontaneous arousal. Around the time my symptoms started, I had a minor accident. During an erection, I heard a "pop" sound followed by a sudden loss of erection. There was no pain, swelling, bruising, or deformity. A flaccid penile ultrasound was reported as normal. I have depression and am currently taking Venlafaxine. I have consulted two urologists, and both felt my symptoms were mainly psychological. They did not recommend a dynamic penile Doppler, stating it would not change the treatment plan. My sexual function is still significantly different from before and is affecting my quality of life. From a psychiatric perspective, could depression or Venlafaxine fully explain these symptoms, or do you think I should pursue further evaluation with a urologist?

Anxiety disorder

I was on petril beta 20 for 3 months and cloba 5 mg in half for one months and betacap for 1 months and after that I stopped eating medicine and within that period I had a severe symptoms like head going to explode like electric shock going on within that 32 days I eat 5 days antidepressants medicine pari cr 12.5 mg and 2 tab of clonotril 0.25 but I want to stop benzodiazepine medicine should I still continue eating clonotril 0.25 so I am very scared I don't know what to do now my head feels like tight please suggest me how can I live without depending on benzodiazepine
