I have taken dveniz 50 for 2 year, d venlor 50 for 2 year, velnof er 50 for 1 year but recently changed to generic desvenlaxine 50 since 15 days , iam getting constant feeling of fear and anxiety in chest , always afraid and worried.
what could be the reason and solution for this problem?

Query regarding Mirtazapine side effects
Dear Doctor,I feel like my ability to distance myself and observe my own thoughts—specifically my metacognition, or the "observer effect"—has become very weak. Even when I practice mindfulness, I am unable to experience this state. In the past, I used mindfulness very effectively to manage and tackle procrastination.Could this difficulty be because my DLPFC (Dorsolateral Prefrontal Cortex) is not functioning properly?Also, how does the Mirtazapine affect my metacognition? Ii don't use this medicine now almost two yr only take one dose 7.5mg that really spike my anxiety level high i stopped it understand that at a low dose, Mirtazapine causes a histamine-1 receptor blockade, which might slow down dlPFC activity. Is there any way or alternative clinical method to reverse or clear this specific receptor blockade?Finally, are there any specialized medical checkups or diagnostic tests available to evaluate the current working condition and functional state of my DLPFC

Escitalopram withdrawal - developed MCAS
I was on escitalopram for 4 years , I weaned off the course and took 2 years to exit . While doing so I developed food & medicine allergies, like itching mouth, eye lids , nostrils, burning in abdomen, and brain fog. Consulted many doctors, didn't find any remedy. Now I can't even take the same old escitalopram or any psychiatric medicines or supplements, all showing up with the same symptoms, everything is connected to histamine.
It is suspected SSRI withdrawal induced MCAS or Histamine Intolerance, still managing with antihistamine, is there any precise treatment instead just symptomatic

Can't fall asleep easily.
Im unable to fall asleep even though my eyes are tired and my body is exhausted. All sorts of thoughts keep on flying in my brain, keeping me awake all the time. Those thoughts aren't negative or depression related thoughts, just random stuff. I lay in my bed for hours are still can't fall asleep.

Psychotic depression
My wife has psychotic depression. It started on Feb 2022. That time she took treatment (oral medicine) for more than one year and completely recovered and had a normal life. After that again on Nov 2024 she developed same symptoms of psychotic depression and then took medicine but this time she did not complete her medication as she recovered and became normal. Just like more than 5 times she develops the symptoms and gets medicine till the symptoms stays. After becoming normal (after 20-30 days) she lefts medicine. Now also she is having the same symptoms and having medicine. So how to stop the psychotic depression coming again and again. Please help.

Hello Doctor,
For over 20 years, despite psychiatric treatment, I have had episodes of burning in my head, palms, feet, and near my ears, along with a urge to pass motion. My hands and feet may suddenly become cold, followed by intense fear/panic and an urge to escape. These episodes are triggered by minor stress, sad memories, or sometimes occur without any trigger.
I also feel detached from reality, as if I am living in a dream and not fully conscious. I feel persistently depressed, negative, and unmotivated. I constantly worry about losing my parents and often wonder how I will cope with life.
I have taken Ativan, Tryptomer, Nexito, Venlor XR, Olaset, Prexep, and Serta, but my symptoms remain. I feel better only when sedated, which is not practical for work.
Could these symptoms be due to anxiety/depression alone, medication effects, or another medical problem (heart, diabetes, thyroid, vitamin deficiency, liver, kidney, or nerve disorder)? What should I do?

I feel this strange constant feelings of fear and worry in my chest without any strong reason. Always afraid and worried idk why. I am also always in a panic state (sort of) like my nervous system is always on alert I react quickly and turn my head around really fast, etc. and then i calm myself down by breathing slowly but the feelings are persistent and stay.
I also feel stuck in repetitive thoughts and no matter how hard I try I cannot escape, the only relief I get is when I am done thinking and for some time I feel relieved only to be stuck in another loop later.
I also fidget a lot and that makes me hard to focus on anything, I stop it forcefully and focus but then I go back to doing it again.
The only things that help are breathing slowly (temporarily) and listening to a static tone like constant tone of a bell or similar.
What is happening with me? Why am always afraid? Are these all related to same thing or are these different problems? And do I need medicine?

What should I do? Should I just die?
I took psych meds, Escitalopram and clonazepam in 2020 bcz i was stressed, then i quit it after three months bcz i became hypomaniac. since then i have mood instability, memory issues, physical pain, and no attraction to girls.
Now in 2025 i again consulted Psychiatrist, he said its GAD + bipolar and its tricky to treat and now i am on 5 psych meds and still have zero libido.
Wtf should i do, should i just die, I am 28 and i am just an mbbs not mbbs md, and dont even have good job.
Also i have IBS kind of problem now, where i am always bloated and fart more than usual, who will stay with me, i think i will always be lonely.
How is it possible that just social anxiety and usual exam stress got converted to such a big problem, i never had bipolar episode when i am not under influence of ssri, but when i take ssri i become hypomaniac. How is it possible, could it be that after tapering in 2020, my brain went into new state of low dopamine and low serotonin low gaba?
