I have severe anxiety with frequent panic attacks. I also have significant mood swings and several other symptoms that make me concerned about bipolar disorder, not just mood swings alone. I have never been formally diagnosed, but I would like to be properly evaluated for bipolar disorder or any other psychiatric condition. I have also experienced obsessive thoughts/behaviors and have had suicidal thoughts in the past.
Whenever I cry intensely, become very anxious, or get angry, I sometimes become unconscious for about 3-5 minutes. During these episodes I can hear everything happening around me but cannot open my eyes or move my body. I breathe normally.
I also have persistent diplopia (double vision). I underwent eye surgery, but it did not improve. My ophthalmologist told me the cause is likely neurological rather than an eye problem.
Recently, I accidentally overdosed on 200 mg amitriptyline.

I think there may be a primary underlying problem, with all the other symptoms being associated with it. The episodes of loss of consciousness may not represent true loss of consciousness and could be related to dissociative symptoms.
I would recommend consulting a psychiatrist for a comprehensive assessment to identify the underlying cause. Once the primary problem is appropriately treated, the associated symptoms are also likely to improve.

The episodes where you become unresponsive for 3–5 minutes, along with persistent double vision, also need neurological evaluation to rule out any medical cause. Please also inform your psychiatrist about the previous amitriptyline overdose and history of suicidal thoughts.

It sounds like you’re going through a lot of difficulties now. From what you have explained there are various issues of which more concerning is the diplopia and the episodes of unconsciousness. While its good that you have sought care for it already, a neurological cause has to be ruled out for both. Meanwhile you will also need to undergo a psychiatric evaluation for the panic attacks and anxiety. With that amount of anxiety I understand you worrying about it being bipolar disorder or not, however these questions could be answered from consultation with a psychiatrist who can assess you and then give you a way forward.
