60-year-old female with recurrent abnormal urine routine tests (2–3 episodes) despite urine cultures not showing bacterial growth.
Current urine routine:
* Pus cells >120 WBC/HPF (normal 0–5)
* Leukocyte esterase 3+
* RBC 10–13/HPF
* Blood 2+
* Nitrite negative
* No crystals/casts
* No burning or other significant urinary symptoms
Previous urine test also showed 25–30 pus cells/WBC per HPF.
Current urine culture interim report shows no organism isolated so far, and similar culture-negative episodes have happened previously.
What could cause recurrent high pus cells/pyuria with negative urine cultures, and which specialist should we consult? Should this be investigated further before giving repeated antibiotics?

I conduct lab tests for body checkup by blood samples every 6 months. My B12 is consistently around 180-200 pg/mL range for last 3 yrs.
This time too B12 was around 200. My CBC also shows low RBC (4.08), high-normal MCV (100.9), high MCH (33.5) and elevated RDW (14.8), although Hb is still normal at 13.7.
I am a vegetarian, so may be miss out on B12 from other foods, thus consistently low B12 may be. What should I do to manage this? Should I take any supplementation?
