I had a prolonged period last year of illness which resulted from an unknown viral fever, dengue, and also lead to further complications. It lasted 6-8 months and continued into this year as well. I need a medical certificate for this to claim academic compensation. I will provide proofs of my illness and the format of the letter. Preferably from a doctor in Pune, but I'll manage if it's not possible. I need it by today itself. Thank you.

I have some allergy issues and I am currently using Montelukast tablets for allergy occassionally but there is no relief. I also use Glimepiride tablets, Amlip 5 and Gitagliptin+ Metformin tablets for diabetes and BP. I also have itching in the eyes and in the body. Normally my pulse rate has been around 85-90 but it easily crosses 100 and last month there was a sudden spike to 130.
