I am having pain in my d only after touching it i have no swelling redness or any pimple or any discharge only a minimum pain while touching it under my paints

I had csection infection before 2 month got restich after that inflammation is there today i saw black worm in urine and some black pieces this comes when i drink large amount of water

I feel My scrotum has loosened since the day i lifted something heavy at home. My sack hangs most of the time. It doesnt feel like before. On touching i dont feel any extra liquid, just the hanging and some semi solid mass. What is it? This happened earlier also but it used to tighten on its own. This time i feel discomfort( not any pain) and it hangs most of the time. No such enlargement is there but its just a bit more hanging than normal. Please advise if this can be remedied at home or i should consult a hospital

Before two months I had very frequent urination , urine culture was negative Uroflometry was normal Kidney function test Normal But scan shows , urinary. Bladder wall thickness 4.5 mm And mild chronic cystitis .. Dr prescribed me  1 niftas 100 g tablet daily for 3 months , but I couldn't take more than 20 days due gastritis , other digestive issues , But it showed me good result , But I stopped taking tablet after 20 days Almost after 2 months now again I feel the symptom is starting again, I don't feel the satisfaction after passing urine , Is taking antibiotics the only solution for this what should I do ?
