Piles surgery

I have done laser haemorrhoidectomy for gr-IV piles in year 2022.The surgeon has ensured that pile mass will shrink completely but it didn't,it just reduced in size. The main issue with my piles is that stool didn't easily came out even after on laxative. So I failed to empty my bowels most of the time. Please provide me the probable cause and solution for the same, I'm feeling very depressed 😔

Asymptomatic gallstone surgery

Hello Doctor, I am a 25-year-old female. I was diagnosed with appendicitis 50 days ago and remained untreated for 15 days. One month ago, I was admitted to the hospital with walled-off appendicitis and was treated with IV antibiotics. I was scheduled for surgery today to remove the appendix. During preparation, my doctor also suggested removing my gallbladder, as I have asymptomatic gallstones (accidentally found) and a family history (my mother had symptomatic gallstones). I have no symptoms related to gallstones — no pain, nausea after meals, or digestive issues. I am worried about long-term side effects of gallbladder removal and would like to know: 1. Is it necessary to remove my gallbladder now, even if it's asymptomatic? 2. Can I safely delay gallbladder surgery and remove it later only if symptoms develop? Please advise based on your experience. Thank you.

Recurrent Anal Fissure

I’ve been dealing with a recurrent anal fissure for 1.6 years. It reoccurs every 2 to 3 months, always at the 12 o’clock position, with a small. Till now i used ayurvedic oil. Currently, my bowel movements are soft. For the past 3 months, I’ve been using Cremagel (diltiazem) at night & Smuth cream occasionally in the morning, applied inside the anus with an applicator. 3 weeks ago, after diarrhea, a redness appeared on the outer skin near 1 o’clock. It looked like peeled skin with fresh new skin underneath, causing discomfort for one day & healed nxt day. The fissure cut at 12oclock is no longer visible externally, and my only symptom is mild burning for 2–3 minutes after motion. Is it safe to continue these creams longer? How long can I use them? Could there be longterm side effects? healing takes around 6 months but my doctor insists proctoscopy examination, saying he cannot provide further advice without physically examining me. I'm scared to put that tool as it may reopen fissure
