I’ve been dealing with an anal fissure for the past 1.7 years. It tends to recur every 3 months, always in the 12 o’clock position, and usually appears as a small cut that heals within a week. I drink around 2–2.5 liters of water daily, and my stools are generally soft.
Initially, I used Ayurvedic oils daily, and for the past 1 year, and from last 5months I’ve been applying Cremagel (diltiazem) twice daily and occasionally Smuth cream in the morning. This has helped with healing, but the fissure reoccurred again yesterday after I had one episode of loose motion the day before. Like a very small cut.
Most of the time, I experience a burning sensation for about 5 minutes after passing stool.
Could you please advise on how I can heal this fissure permanently without surgery and recommend an ointment for long term use or any fibre supplement?

Definitely we can try conservative management for one more time maybe. But in your case the best option is for surgical correction. Fissure surgeries are not major surgery, they are only minor surgeries which can be done on day care basis. So no need to be afraid of surgery

By diet modification itself u can happily feel comfortable
But usually trigger factor elimination maybe difficult to follow and so permanent treatment is to be given
