Hi sir . It seems u have acute on chronic fissure may be with a sentinel pile also known as skin tag . More than medication u need to change diet habits . Take plenty of fibre food I.e green leafy vegetables and vegetables . Avoid spicy and non-veg food for few weeks. Do a sits bath daily for 5 times . Coming to medication u can use Syr lactilose 15 mL at bed time which helps as a stool softener and decreases straining at stool. For pain u can use ultra st tablet if needed. Use ointment ANOBLISS , apply a peanut size ointment over the fissure . Get your thyroid profile done .

If you have tight stool then start taking stool softener or increase water intake
If stool consistency is ok then avoid excessive straining ,use western seat .

while sitting try to do false holding back exercises like imagine you have to control your stool or pee and you are in public place
try doing this for 20-30 seconds
Repeat this exercise 5-6 times a day
get checked by general surgeon
If it's chronic anal fissure , surgical management is best option - lateral anal sphincterotomy
For anal prolapse -will need further evaluation to find out if it is haemorrhoids or rectal prolapse
