I have fatty live since 2016. My weight is 100 kg. I had grade 2 in 2016 and now it is grade 3. What should I do and what medicines should I eat. I eat milk thistle to reduce my fat but I had hard time reducing my weight is very hard to reduce.

I have injured in my right elbow joint, in xrays it was found that a crack in bone and the doctor advised  for surgery. I undergo surgery and doctor fixed 2 screw. Both xrays are attached. My all finger and other activity is ok. Had laparoscopic appendectomy 3 days back. Today I had abdominal pain and felt like gas trapped and nausea. Haven't passed stool since surgery but have passed some gas today.
