Brain and Spine Problems

Pain in right shoulder joint(from one year) . And Pain from right neck and it continues to last two fingers of the right hand and Pain in upper right back below the big bone(from the past six months). Took medicines nearly 6 months as prescribed by orthopedic and recently consulting neurologist and taking medicines as he prescribed. Still suffering and effecting both my professional and personal life. Please advise. (I am a working professional and sits in front of the computer nearly 8 hours a day with breaks.) please advise the treatment. Will the physiotherapy cure my problem?

I always experience a throbbing headaches in the morning which at times, accompanies with Nausea too. I have it in the back of my head. The headaches lasts up to noon. And also I don't have a wrong sleeping position too. Then what could be the reason?

History of hypertension n diabetes from last 6yrs.. Recently experienced high bp 215/98 blurred speech n loss of appetite with weakness.. Medication for HTN n DM os on & off. MRI shows acute infarcts in basal ganglia. Chronic lacunar infarcts, ischemic demylinating changes in bilateral periventricular whitr matter region. Cerebral atrophy.. Having constant headaches, near eyes. Got my eyesight checked few days back and am wearing proper glasses. It is happening very often these days.
