Tooth and Mouth Problems

Hi, I got gum problems and severe bone loss in teeth. I want to know if any periodontist provide gum chart (periochart) to analyze how much gum loss occurred on all the teeths. If anyone knows of dentist who provides gum chart please help me with contact details. Thanks.

Whenever I have something sweet like. Coffee or tea or whatever it is, pain starts in my teeth, especially when I've something at night time, my tooth and mouth begins to pain. I'm not able to sleep also and my mouth swells.

I had my wisdom tooth extracted through surgical method before 9 months and now im having pain in my jaw bone. I can't even open my mouth fully. The Pain extends to the head and ear.
