Skin, Hair and Nail Problems

Hi, For the last one year I am feeling rough textured skin on the butt and there is a muscle loss as well. When I took a picture, I could see the blackness of the skin. Sometimes feeling itchiness. There were mild fungal infections earlier and had treatment with the creams. Medical history asthma, allergy and gastritis. I have attached the picture, kindly look. Is this anything to worry about?

I have this nasal crease on my nose from years. Had a DNS Surgery to get rid of nose blockage and allergy also decreased after that. But the crease is still there which happens due to repeated rubbing of nose in upward direction. How should I remove it?
