I have done my ultrasonography of the lower abdomen for irregular periods. Can you please go through the report and tell me if there's any concern regarding PCOD / PCOS/ other abnormalities?

I am shameem my age is 33F trying to conceive My amh is 3.1 .I had regular periods of 26 days menstrual cycle.I went for a follicular study for 3 months I was given letrozol,hmg injections,HCG injections, progesterone but i didn't not conceive in those three months.In the 3 Rd month I had black muddy periods. This month I stopped consulting doctor i did not take any medicine today is day 18 of my menstrual cycle I am bleeding. Why am I bleeding in the middle of the month. My periods are suppose to come 11 days later but why do I have periods now?
