Chest heaviness, balance problem

Hi doctor i am male of 27 years, i feel pain in left arm some time in back shoulder and left chest, 7 months ago i did some routine test found my Ldl was high, TG very high, Lp(a) 165 i did ecg came normal and doctor advised Stress echo which came normal 7 months back, I have been smoking from last 3 years and drinking daily, from last 1 year I feel sudden diziness, floating sensation and spinning and ear ringing with breath shortness and palpitations. Also from last 4-5 days i am feeling heaviness in centre of chest and upper abdomen Do i have any heart issues? As 7 months ago stress echo done be effective even now

I have light chest pain and shoulder pain, and I have done heart related tests like ecgs , 2d echo, troponin I and TMT , all results are came normal but slight high cholesterol levels like total cholesterol 230 ldl 160 hdl 42 triglycerides 180, I am 95 kgs and aged 28, trying to reduce weight too can I do gym? Is the above report is ok can i do my regular activities and i didn't understand about my IVS-21MM , PWD 20mm
