Is the surgery must be required for this full thickness tear off of distal supraspinatus tendon or is there any other way to diagnos the injury? MRI report is attached

Accessory muscle noted in right chest on HRCT. Pain radiates to back of shoulder with burning sensation. shoulder becomes weak. I have tried physiotherapy too but nothing worked. Any treatment for this ?

I had a fracture surgery 2 months ago but now when i scratched at my injured site a small layer of skin got pulled and after then it started producing puss and transperant type serum and sometime blood. Its been 6 days Today i done crp it is in the normal range 2ui/ml Is it any type of infection or bone related infection or its just a skin irritation . Can a skin irritation cause puss and serum discharge. And if crp is normal then there is no infection or what?

Had a back strain while lifting heavy 40kg weight . Had an instant pain and lower back spasm after that. lower back muscle tightness and pain went away after 5-8 days. I am doing back exercises and push-ups now but after 2.5 weeks still have a localised nagging pain on left side lower back.
