I had a cavity in one of the upper premolar tooth on my right side and got a filling. After some time filling dropped and cavity incresed. This is like 4 years ago. I do not have any pain in that but feeling like it is numb and some times I am getting right side head aches. There is gap between teeth and I see it is black on that tooth. Please advise what treatment is needed? Is it root canal or filling or extraction. I get numbing sensation around that tooth most of the time

I had a history of plaque deposits since birth which was cleared through dental visits. Since then I have been taking proper care of my teeth.
