Tooth and Mouth Problems

Due to continuos chewing of tobacco mouth not open properly ....due to this problem the problem occurs while eating food or meals....

I have a cavity in the upper right pre-molar tooth. Earlier I had visited dentist twice and both the times GIC filling were done. However I have noticed that the GIC filling comes off within 3-4 months. Is there any permanent filling process? Preferably without root canal / crown?

I am having pain In my wisdom tooth. I can also see some black spots on It. I don't generally feel any pain otherwise but When I am touching that black area with a toothpick I am feeling some sensitivity In It. I just want to make sure If the pain Is due to the wisdom tooth or cavity ? Could you please help me out?

Today my molar broke off at the gum line. A root canal was started on the tooth sometime ago due to severe decay which was into the nerve. The temporary filling fell out and caused the tooth to crack so was told it needed extracting. Will this make the extraction harder? What can I do to help the throbbing pain which start when the took broke off?
