What test to be done to detect the issue causing blood while pooping and what are the reasons for this issue?

Hi . My son is 5 years old and from past 4 days he is having constipation.. with great difficulty he passes stool which is very hard and painful for him.... we gave him soft foods, apples , banana and all possible fiber rich food. Don't know what medicine to give. He has congenital hypothyroidism and is on very low dose medication(12.5 mcg) . Please suggest a solution accordingly.

I had a c section before 5 months.I am not able to have regular bowel movements afer that .I had to take kayam churna or softovac for that.My weight is also increasing day by day.Pressure is created but I feel constipated.Kindly prescribe some medicine for that.
