Clinical Presentation: Morphology: Approx 3cm circular patch. The center is smooth. The border shows roughness, which appears to be short, broken hairs. Sensory Deficit (Crucial): I have tested the area with thermal (ice) and mechanical (pin) stimuli. The patch shows definite Hypoesthesia (diminished sensation) compared to the surrounding scalp. It is not totally anesthetic but significantly duller. Associated Symptoms: No itch in the spot itself. However, the rest of my scalp has severe dandruff and pruritus (I recently stopped using shampoo). My Concern: Given the hypopigmentation and sensory loss, I am worried about Hansen’s Disease (PB) vs. Alopecia Areata vs. Tinea Capitis. Questions: Based on the morphology (smooth center, broken hair border), does this look more like autoimmune or infectious? Does the sensory loss warrant an immediate biopsy/slit skin smear? What is the recommended immediate course of action?"
