Skin, Hair and Nail Problems

I have a severe Itchy skin and to manage I take Cetirizine 10 mg once a day from last 5-6 years. If I skip it for a couple of days then my whole body itches so bad that I can't stop scratching. Moisturizing skin a few times a day also never helped. What could be wrong with my skin? What to do?

Have been suffering from chronic angular cheilitis for about 10 years. I got fever overnight, the fever stayed somewhere around 12 midnight-1pm. I washed my head and took a sponge bath and gradually the temperature came back to normal. No fever, cough or cold thereafter. During fever only, in the morning when I woke up, I found the redness/patches in my hand (between elbow and palm). No such thing in entire body. Two big patches on each hand followed by some smalls. Itching at times. It's the 4th day. Visited an general physician and he prescribed mupucip ointment, which I am applying daily twice. Mam/sir  since childhood my hair used to very silky and smooth and thick as well... When i reached class8, i lost half of my hair and gradually my hair became very dry, rough, frizzy, thin... Now I'm 17 years old and still my hair are thin, frizzy and since one month I'm having a lot hairfall...whenever i comb my hair i see hair...I'm scared to wash my hair because it causes a lot hairfall...whenever i get period its only for 1-2 days... Gets less on 2nd day and then gradually almost equal to zero... I tried so many things but nothing helped... Like hair oils, hair serum... Antihairfall shampoo... But nothing really helped...
