Skin, Hair and Nail Problems

I've these pores kind off thing under skin which is white. It's visible white from outside and hardens nearby skin and peeling off the the first layer of skin I see these white pores which won't come out and it's roots are kinda inside. Over the time it keeps skin thick and make it hard. I've had this before probably in different finger and when it came out it had white hard pores inside the skin. Kindly suggest what this is and why it's happening in different area of skin specially in palms.

There is some kind skin infection on tip of my thumb. There is pain when the nail grows over it. It's pains even when pressure is applied over it.
