Hello Doctor, I am having pain while swallowing food and even while drinking water for some time. I have already undergone tests like endoscopy (UGIE), HRM/manometry, CT thorax and gastric emptying study. Endoscopy and CT were normal, but manometry showed ineffective esophageal motility. I also have a history of reflux/GERD and hypothyroidism. Currently my main symptoms are: Pain while swallowing food Pain while drinking water Mild throat/chest discomfort and anxiety due to pain No vomiting, no breathing problem, and bowel movements are normal. Recently I also noticed a small painful lump/swelling below the ear/upper neck area. Current medicines: Thyroxine Itopride Antacid/alginate syrup BP medicine (Telmisartan) Please suggest what could be the possible cause and what further evaluation or treatment is needed.

Hello Doctor, I have been taking Paroxetine CR 12.5 mg at night for the past 3 months for anxiety and mild depression. Mentally, I am feeling much better. However, I am facing some issues: * Irregular bowel movements: frequent loose or semi-loose stools, sometimes normal. Urge to pass stool after eating. * Anal itching/discomfort: constant itching and irritation in the anal area. * Daytime drowsiness: difficulty waking up before 10–11 AM even after adequate sleep. * Weight gain over the past few weeks. I have attached a stool image for reference. My questions: 1. Are these side effects of Paroxetine? 2. Should I change dose/timing or switch medicine? 3. Can I take probiotics for gut health? 4. Do I need any tests? 5. How to manage drowsiness and weight gain? Please suggest treatment or lifestyle changes. Thank you.

I have too much pain in the upper abdomen radiating towards the back and causing me lot of discomfort and sufferance from past two weeks. I am having severe GERD with esophagitis grade B and fatty liver. I do have reoccurrence of hiatus hernia. I am a Bariatric patient completed my sleeve gastrectomy in the year 2020. Bcoz of the GERD complexity I have got ineffective esophageal motility also in manometry. Doctor suggested me for a roux en y gastric bypass as options are limited for me with a long term PPI use which is presently not functioning on me. But suddenly from past two weeks I have severe stomach pain radiating towards the back and getting sticky stool which is not going easily from the WC. Yesterday I did USG whole abdomen and they found infiltration of Pancreas where they couldn't see the tail as it was obscured by bowel gas. Please suggest as stomach is in bad pain which is getting difficult to manage.

Swelling posterior half right side anal margin started on 28/12/25. I could feel hardness only on pressing area there was no obvious swelling  On 30th i could clearly appreciate swelling. There is worm crawling feeling and sticky fluid discharge after bowel movements. I can walk.. run..from begining of pain. I sat for 1 hour on cushion chair.. metal chair causes pain,I just can't sit on yoga posture with folded legs!very uncomfortable. Pain is very sharp on coughing and after defecation... No improvement at all here even after 5 days. Whenever I cough I feel severe unbearable pain ..then I lie down on bed so pain settles. I can sleep without any discomfort for begining. Is it perianal abcess or thrombosed external piles based on history?

I have been experiencing upper back issues for over some years. In 2025, I had severe fatigue in my upper back, along with tightness and occasional spasms. After doing regular strength training and physiotherapy for the last 4 months, the fatigue has completely resolved, but the discomfort and tightness still persist. The symptoms are episodic: when the tightness comes, it lasts for many days or weeks, and when it subsides, it stays away only for a few days. When the symptoms goes I feel a very pleasant, relaxed sensation in my back in that process. However, when the tightness is present, I experience noticeable shivering/tremor in my left hand. A few months ago, before this tremor started, I had brief episodes where my fingers would momentarily “lock” for a second. Because of this, I consulted a neurologist and underwent an MRI (spine/brain), which was reported as normal. Kindly suggest me some hospital in Visakhapatnam. I currently stay in a near by town.

Im attaching a picture of my thumb. So i have a very small hard lump inside my thumb since childhood. It dint bother me much. The skin on top was always normal and i was able to live normally. But since a month the inside lump i feel grew a little and almost around the inside lump i see this yellow appearance on top of thumb currently. Went to dermat last week, she said its a corn and gave me salicylic acid cream which i used for few days but no improvement. The yellow thing is slightly just growing. I went to ortho today. Took xray it looked normal. He said use the salycylic acid cream few more days. Im really not happy with the diagnosis because i dont think its a corn at all. It does not pain on its own. Only discomfort, slight pain when i do something that puts pressure there. The inside lump since childhood is causing these issues now i feel. Please guide me anyone. What is this? Do i need to go to a dermat or ortho or hand surgeon.

I’m a 26-year-old woman, and for the past three years, I’ve been experiencing persistent pain in my lower right abdomen. It initially started near the right side of my vaginal area and has gradually spread upwards toward my belly button. I’ve noticed some swelling in that region. The pain is generally bearable but varies in sensation—it can feel like burning, sharp needle pricks, or cramps. Occasionally, I also feel discomfort radiating into my right thigh. I also experience numbness in the affected area when I touch my abdomen with fingers. Lately, I had a lymphnode on my neck and I feel I got misdiagnosed of tuberculosis, I completed my medicine but this pain was always ignored by that doctor. Despite multiple visits to different doctors, my concerns haven’t been taken seriously, and I haven’t received a clear diagnosis. I’m planning to get married in a year, and I’m increasingly worried about how this ongoing issue might affect my ability to conceive in the future.

Hello doctor, I have IBS-D and I notice a very regular pattern linked with my menstrual cycle. About 10–12 days before my period starts, my PMS symptoms begin — I get nausea and frequent loose motions (3–4 times daily). This continues until my period starts. During my period, I again experience diarrhea, nausea, and stomach discomfort. Even after my period ends, the symptoms don’t completely settle — for a few more days I still feel nausea and loose motions. Another problem is with my appetite. A day before my period, my appetite increases a lot, but on the first day it suddenly drops. For the next 5–6 days it stays average. From around the 7th day after my period, I completely lose my appetite.. I don’t feel like eating at all. Even if I try, after just 2–3 bites I feel full. Because of this cycle, I am unable to gain weight and in fact, I am losing weight. This is causing me a lot of stress. Could you please guide me on this?
