My father got 2 small attack on 22nd Nov and by same night he got major attack. After 36 hours around patient has been shifted from dhanbad to Kolkata hospital. On 28th angioplasty has been done. he had 90% blockage in main vessel. One stent they have put..he got discharged on 1st but again he has been admitted in hospital on 2nd as water has filled near lungs and heart and he was not able to breath at all. He was in CCU for next 7 days and he got discharged on 13th Dec. Recently blood test has been done for BNP( BNP was 956), potassium 2.4 serum creatinine 1.84..doctor has changed the medicine for 2-3 days...now he is taking concor 1.25 mg, prax 10 mg, eptus 50 mg, ivabrad 5 mg, ecosprin 75 mg, aztor 10 mg, potklor, febutaz 40 mg as uric acid was high, ...but since yesterday he has difficulty in breathing, he can't sleep...what to do?

I played table tennis for 20-25 mins yesterday after 2-3 years. I don't generally have a workout just probably a walk of Hardly 1 km. After playing game I heart beat remain elevated for couple of hours around 100-120 sometimes reaching to 145 then coming back 110. I also had weakness in hands and legs and generalised weakness and anxiety. My oxygen levels remained intact at 98. I took concor 2.5 mg and slept in the morning I had HR of 70-75 but was feeling weak. But just 15 mins back I feel dizzy and my HR raises to 145 then came back to around 80-90 still in this bracket. Am I having any episode of ventricular tachycardia or what ? I am very afraid. I had coronary angiography in the past it was normal only myocardial bridge in mid LAD ? Should I be afraid of impending cardiac arrest or is it just a sinus tachycardia?

Dear Doctor, I am 42 years old unmarried man. I am obese 110kg weight and 5 feet 6 inch height. I get pain either in my left arm, right arm chest,  legs for the past few days. I checked ECG few days ago in a Lab, report is normal. Echocardiogram done in July 2019 report is normal. Did Master health check up few days ago report is normal. Having Concor 5mg for high BP, high BP happens only during stress. Mostly my BP at home is 110/ 75. Only on exertion or during mental stress BP goes up. Heart rate goes up to 175 BPM on walking fast. Resting heart rate is normal. Having medicine Nexpro L for GERD. Why am I having pain in chest, left arm, right arm legs, lightheadedness? Note pain not at same time. Sometimes left arm pains a lot. Sometimes right arm. Toes fingers pricking sensation. Shoulder pain. Sometimes chest pain all this at rest. Slight sweating at times. I fear whether there is a possibility of getting heartattack? I want to do CT Angio. Please advise.

I am 42 years old unmarried man. I am obese 107kg weight and 5 feet 6 inch height. I take medicine for GERD Nexpro L and Nexpro 20mg, for Asthma Montek AB and Formoflo inhaler, Asthalin 200mg rotohaler, High BP, High heart beat on walking upto 175bpm, and Cetrizen for urticaria. Taking Inzofresh 10mg for sleep. I have severe ED for the past 8 months. I am having Concor 5mg from October 19, given by cardiologist. I have done Echocardiogram in July 19 by a Cardiologist. Report is fine. I have done Endoscopy in January 19 by a Gastroenterologist report is fine. My HDL is low at 35, master health check up done in December 19. Am I having heart blockage? Why ED problem? I don't do any excercise or walking. I am doing online business from home. My acid reflux is causing throat irritation, phelm feeling, chest discomfort chest muscles pulling sensation, pricking pain in chest, stomach burning sensation. Is there any need for another endoscopy? Is there any need for Angiography or CT Angio? I

I was diaognised in2D echo and colour Doppler as - LEFT VENTRICLE : ASH, AORTIC VALVE : Trileaflets and thickened, and COLOUR DOPPLER : AV GRDT 5mmHg, AR GRADE 1. E/A REDUCED. Conclusion: :HYPERTENSIVE HEART DDISEASE :ASYMMETRICAL SEPTAL HYPERTROPHY :SCLEROTIC AORTIC VALVE WITH MILD AR :LV DIASTOLIC DYSFUNCTION :NORMAL LV SYSTOLIC FUNCTION (EF - 64%) :NO REGIONAL WALL MOTION ABNORMALITIES AT REST. For the above issues kinldy advise 1.Whether it is curable/controlled. 2.Whether surgery or implants in any way will be of any help. 3 Your considered opinion,suggestion/solution. I am non diabetic and normal heart rate 50_60 bpm regular. MEDICATION: Zabeta X in morning for nine years. Later changed to LODOZ 2.5 for a year. And now further changed to CONCOR 2.5 for the last 15 days.( Restyl tablet 0.5 is being taken at bed time since 9 years.)

Hello Doctor, I am 45 years old unmarried man. I am 130kg in weight very obese. Height 5 feet 6 inches. I did blood test today. My fasting sugar is 102, Cholesterol is normal. My Heamoglobin is low at 10.8. My RBC is low at 4.15 million/ cu mm.Kidney  My creatinine is 0.79 mg dl, Liver My bilirubin is low at 0.26. Why is My Heamoglobin and RBC  low ? I am having Livogen medicine 1 tablet daily. Despite that it is low. Why my creatinine is low? I am having medicine Concor 5mg and Abel 40 mg daily for High BP. Taking medicine Montek AB, and Foracort Rotohaler 200mg. I am having Tadaflo 5mg daily for poor erection. I am having Gressami50mg Quietegress 50mg and Skizosmooth. My erection of penis is poor despite having Tadaflo 5mg. I get pain in hip waist on walking for 100 meters and if I stand for 10 minutes. Please advise treatment for low Heamoglobin and RBC low creatinine.👈

Dear Doctor, I am having for the past 1 year chest discomfort, like pressure, burning sensation, lightheadedness, left fore arm feels cold. Now my ankle is swollen. Feet pains on walking. I have done Echocardiogram in July 2019 report is normal. I did ECG on 15th July 2020 report is normal. I at times get high BP then it becomes normal. Heart beat goes upto 175 on walking fast for 1km or climbing many steps. Having medicine Concor 5mg, for GERD Nexpro L, Asthma Montek AB. Uriticaria having cetrizen. Inzofresh 10mg for insomnia. Endoscopy done in January 2019 report is fine. I still feel pain in Esophagus, at times reflux. Master Health Checkup done on 15th July 2020 report is fine. My question am I having heart blockage or Angina? I am unable to do TMT because my heart beat goes up within 1 minute of walking on TMT. I get panic often due to fear of heart disease cancer etc. What is my health problem? Please advise which Doctor to consult?

Dear Doctor, I am 42 years old unmarried man. I am 107kg in weight with big tummy and I am 5 feet 6 inch tall. I have been told I have GERD and asked to have PPIs. I am having Nexpro L and Nexpro 20mg for the past 9 days, I stopped it because of less symptom. I have asthma, I am taking Montek AB and Formoflo inhaler. Uriticaria having cetrizen daily for the past 9 years. High BP increasing heart beat on walking climbing steps upto 175bpm taking medicine Concor 5mg. Endoscopy done in 2019 January report is fine. For the past few months I have been eating spicy food from hotels, sweets. Now I am getting reflux without eating food I mean in empty stomach, throat I feel like phlem, sometimes sore throat for the past few months. My fear is I must not have Barrett esophagus due to repeated reflux. Would Asthma inhaler Formoflo 250mg cause GERD? What is the solution for this problem? Why throat is irritating? Another endoscopy is required? Please clarify.

Dear Doctor, I am 42 years old unmarried man. I am obese 107kg in weight and 5 feet 6inches in height. I have GERD, Asthma, at times noticed High BP, urticaria. I have done Endoscopy in January 2019. Report is fine. Echocardiogram in July 19 report is fine. My heart beat increases on walking, climbing steps upto 175bpm. Resting heart rate is normal below 85. What could it be the reason? Heart blockage? I have been given Nexpro for GERD. Concor 5mg for high BP. Cetrizen for urticaria. Montek AB and Formoflo for Asthma. For the past few months I am having throat sore, as if phelm in throat, slight dry cough, at times burning sensation. Now I have blisters on tongue and stomach burning sensation. I eat spicy non veg food, cake, and chocolates recently. I know I must not have it but I love food so eat it. Now I am suffering please give me medicine for throat burning, tongue blisters and stomach burning.

I am diabetic since 17 yrs and hyper tensive since 11 yrs, using GLIMULIN MF FORTE in the morning, JANUMET 50/1000 in the afternoon, GLIMULIN MF in the night. CONCOR AM 5mg in the morning, TELMA 40 in the evening, GABAPIN NT 400 twice daily, ECOSPRIN AV 75 in the afternoon. But since last 6 months I have been facing episodes of severe gas trouble with bloating but no pain in the stomach. For this the doctor had suggested RIFAGUT 550 mg twice daily for 5 days, then after the reoccurrence in 1 month he again prescribed RIFAGUT 550mg for another 5 days along with VSL-3 once daily for 10 days after lunch, again there was an episode after 15 days FLUDAC 10 mg capsule was asked to be taken at the night but no cure.For the last 3 months,I am using Patanjali kumayasava at the time of gas trble. I have observed that in all these episodes there is one thing common in my diet which is watermelon/milk/tomatoes,etc. I doubt that it's high fructose level is the culprit.Kindly suggest me what to do.
