Taking ativan 2mg for insomnia for 6 years , recent times struggling with sleep and excessive thinking. Doctor have added escitalopram 10mg along with Ativan, but If I take escitalopram facing gastric and burning during urination on next day. Please suggest.

Dear Doctors, Since a severe panic attack in **Jan 2024** while traveling, I’ve developed an intense fear of heart attacks linked to leaving home. Every attempt to travel (even short trips) triggers **4+ hours of left chest/shoulder pain** and dread of dying, but symptoms vanish once I return home. This cycle reinforces my phobia, making travel impossible despite no cardiac history. **Current Meds:** *Inderal LA 40 mg* (started at 20 mg), *Tryptomer 10 mg*, *Stalopam 10 mg*, and *Zapiz 0.5 mg* (emergencies). **Concerns:** Doses keep rising with no relief. *Is this anxiety-driven? Should I revisit cardiac tests? How to break this fear-travel-pain cycle?* Urgently need strategies to reduce medication reliance and regain mobility. Thank you!

I have been experiencing purposelessness and emptiness from a long time, There is a constant rise of cortisol (which I have presumed as I can't sleep all night and wake up randomly at odd timings) I am a medical student in 1st year in russia, There have already been problems back in india with my family, My mother is diagnosed with chronic delusion with hyper aggression and is going under treatment. My Dad has panic disorder and anxiety for which he is taking medications, Also both my parents are diabetic, And my mother has hearing loss. So coming back to my condition, I have been feeling low from past 2-3 years, Especially after failing in neet, I had attempted suicide once due to a family dispute and constant fights. Now, I thought coming abroad would fix everything, But here as well due to my aggressive nature and foul language, I fail to make and maintain friendships, Going through relationship issues as well, I feel very filled up. I feel leftout & feel like taking alprazolam.

Hi Doctors I need your help in finding the solutions for my stress, anxiety, fear , nervesness and anger. Having this issues since Covid started with health anxiety and day by day it's worsen. Getting too angry which leads to palpitations, chest pain , I'm crying very often. Reason behind stress is that if I don't like or person who don't do the right things I don't lie them and I show anger on them even if I see them I become more angry and anxious. Please help me to stop anger which will avoid crying every day.

Having history of CNS TB, vasculitic infracts & Hydrocephalus treated in 2024. Nightmares- I often see myself dying in dream, ghosts, someone trying to kill me. I feel like screaming loud, crying. I cry for hours even after small argument with partner. Sleep problem as well. Currently taking Eliwel.

He also has a hiatal hernia and GERD. In 2016, he had repetitive thoughts and sleeplessness, managed for four years with Fluoxetine 20 mg, Clomipramine 25 mg, and Lorazepam 2 mg. In 2020, an ischemic stroke (PCA territory) triggered a relapse of depression. Prior meds failed; Fluoxetine rose to 80 mg (no effect), Escitalopram 20 mg (ineffective), Venlafaxine 75 mg with Bupropion 300 mg (helped but stopped due to gastritis), and Mirtazapine (unhelpful). Lorazepam dependence persists despite trials of Clonazepam, Diazepam, Melatonin, and Trazodone. For four years, refractory GERD, on Esomeprazole and Levosulpiride, continues. Currently, he has ongoing low mood, disinterest in socializing and activity, poor appetite, weight loss, and insomnia, with treatment-resistant depression.

Hii I have suffering a problem in which after I woke up I don't remember much, I'm forgetting everything, EVEN I don't remember the names of my office colleagues, and my mind is like it's out of my control I have a headache everytime, feeling heavyness on my head, I don't know why, but I start feeling active after 7pm . The problem just vanished after 7 pm I don't know what is happening Yesterday I took full body checkup It shows severe deficiency of vitamin D & B12 except these all the reports are normal This is happening to me since 3 years Please advise me
