Kidney Problems

Blood was coming in urine 15 days back but after drinking lot of water it stopped coming in 2 days. Now from yesterday again blood is coming in urine with lumps of blood clot.

My father is having Creatnine level of 4.7 and urea 81.50 and has hemoglobin count of 8g/dL , Packed Cell Volume 23.30. He has no swelling in the body. He is conscious but is sleeping all the time ,speaks rarely and cannot walk by himself. What should I do in the current scenario?

My father has a high cretenine level of 4.97 his Urea is also 81.50 Alkaline phosphatase is 142 Protein is 4.68 Albumin is 2.89 Uric acid is 5.00 Sodium is 132.90 Potassium is 3.07 His CBC reports are also not in check as hemoglobin is 8.00g/dL PCV is 23.30 RBC Count is 2.63 His Urin has some protine in it. He is conscious but does not talk often and can't walk aur perform any physical activity by himself. What measures should I take in the current scenario and can this be sorted by medication?
