Hello, my tsh level is 13 while my t3 and t4 levels are normal so kindly suggest medicine is necessary or it can be treated without medicine. What changes should be done?

My mother has a diabities from past 10 years she was first on medicines but one day her levels out of nowhere started inclining from last year later the doc Was prescribed for (insulin wosulin N 10 ml and wosulin r 10ml) she do 3 shots of wosulin r of 15iu and one short before bed 20 iu. Hello dr, around 6 pm in the evening in empty stomach my  blood sugar level reading is  118, is it normal.  I eat chapati /roti at breakfast, rice with veg curry at luch and drink tea at evening time.
