Hello Doctor, I’ve been taking Met XL 25 mg once daily in the evening for the past 7 years. But today after dinner (I had only a simple sweet dish – sheera), I suddenly experienced palpitations and anxiety. I checked my BP at home — it was 170/110 and heart rate was 104. I rushed to a nearby hospital. ECG and cardiac profile were done — both came back normal. At the hospital, my BP was 160/90, so the doctor gave me Stamlo 5 mg (Amlodipine) as an emergency dose. After taking it, I felt better. When I asked if I need to take Stamlo 5 regularly, the doctor said it was only for emergency. Now I just want a second opinion — Do I need to start Stamlo 5 daily or not?

Back in 2018, he underwent coronary angiography which showed 2–3 blockages (we don't have exact percentages documented now), but he opted not to undergo stenting or bypass at that time. Since then, he's been on medication-based management and remained symptom-free. I am currently on a Somatic Symptom Disorder Treatment and recently started medication Escitalopram 5mg and Etizolam 0.25mg in the night. It's my 13th day pf the treatment and have experienced 2 episodes of panic attack within this treatment. During the panic attack my BP spikes to 160+/100 and 90-100 Heart Rate, it causes headache and pressure during the attack in head. Whereas normally it range between 130-140/80-90 and 65-60 Heart Rate everyday. My heart rate being on the lower side of the normal range , I don't know whether to take any Beta Blocker like propanol daily or SOS during panic attacks OR any other good medicine which can fit best in this scenario.

I went for normal check up today. I had done my ECG and ECHO . ECG shows poor R wave progression. ECHO is normal. Am attaching two. Kindly advise. Poor R wave progression is normal ? I have anxiety issues and taking CIPLAR LA20 And Mirtaz 7.5 kindly advise.thank you.
