Need consultation for TSH low level ,need to know the reason and future diagnose to treat She is taking thyronorm 50mg as of now

I am asking this question here again as I have tried consulting several different specialists online and offline and still have no idea what is the issue. I have palpitations that start ONLY when I sit or lie down. And syncope when I try to sleep or in the middle of sleep as I wake up scared and dizzy. ECG/ECHO were normal. CT showed a slightly enlarged thyroid with a tiny nodule but hormone levels are normal. Please advice
