For getting feminizing hormone for a trans gender woman ,what are the lab test need to be done before? The patient has a GID certificate from psychiatrist and is ready for hormonal treatment. Can it be done online?

It's 5day after RTPCR confirms Covid with CT value 23-24. Fever not subsiding from 100.6°F so HRCT Done which has CT Value 5 of 25. Fasting Sugar was 196 took GlimyM2Fort (Metaformin 1000 & Glimipride 2mg) at 10am after breakfast. In lunch took 2 Chapati, Daal, 3tsp rice at 2:30pm, at 7:45 pm random Sugar is 344. Should I opt for Direct insulin as I can't takt more then 2 tab of GlimyM2Fort in a day. If yes, need more info.

I feel dull and lethargic for the past 7 days. I went for a blood test. Sugar levels are 101 fasting and 80 post meals. Thyroid is high at 7.56. Cholesterol 201. All others are ok. Please advise. The whole day I feel sleepy and this is happening only since a week. Thanks
