Hi Doctor I had a random health check up I found Tsh 0.027 FT3 and FT4 are in range then the doctor asked to get USG ultrasound

📌 Key Observations
- Thyroid Size & Texture:
- Both lobes and the isthmus are mildly bulky and show heterogeneous echotexture—suggests some irregularity in tissue consistency.
- Suspicious Nodule:
- A hypoechoic lesion in the right lobe measuring 7 x 8 mm.
- Described as “taller than wider” with microcalcifications—both features associated with higher suspicion for malignancy.
- Classified as TIRADS 5, which indicates a high risk for thyroid cancer.
- No vascularity seen on Doppler (which may be reassuring).
- Lymph Nodes:
- A few prominent but normal-looking lymph nodes were found on both sides of the neck—largest is 18 x 6.5 mm, with preserved fatty hilum and doctor asked to get FNAC I am more anxious about it please help

Circumcision details regarding

Amylase is out of range
My amylase report shows high value 195. Last year I tested it was around 200. Assuming it is chronic & may be exist for years & it was detected last year by chance what could be the issue. I don't have diabetes. Could this cause body itching or eosinophilia or how it should be treated

How can it be detectes high sugar level
What test can reveal the reson for hugh sugar leavel even when on metformin, fenugeek tabs and diet is normal. Is it because insiling produced by the body is not utilized or due the body is not producing insulin. Henerally fasting sugar level is 110 to under 200 and hb1ac last month was 6.3.

About diabetes medicines
Asking for 70 yrs male. FBS 108, PPBS 142, HbA1c 6.10, eAG 128.37 Earlier He was taking glyciphage sr
500 once before breakfast since many years. But last year he got fever and sugar and bp went down so doctor has changed the medicine to glucobay 25 with first bite of food 3 times a day. But now after seeing above sugar reading doctor has prescribed, Glyciphage SR 500 mg before breakfast And Glucobay 25 with 1st bite of lunch and dinner. Is this new change in medicines ok? Can he take these 2 types of different medicines in a day?
Because he started getting gas and acidity issue at night since these 2 medicines has started. And also he doesn't feel like eating at night. So please advise which medicine to continue?
