How do doctors determine  the time for removing tmj splint after that is when doctor decide to remove splint used for tmj disorder?

While drinking water teeth is paining... What should I do. Even after oil pulling.. Teeth getting pain.

I have swelling under my left ear near jaw. Pains when pressed hard. And swollen left submandibular lymph node. Also my left tonsil is swollen. And having a pain right from my lower third molar, tonsil, ear and jaw. My wisdom tooth is impacted and there's tooth decay as well. I took augmentin 625 but no improvement. I have attached photo as well. Is this pain because of the teeth. I am a smoker as well.
