Tooth and Mouth Problems

Few months back my one teeth ( It's in lower incisors teeth) got loose doctor suggested me to wait for 2-3 month it would be settled by itself so I did not do anything but now I am having gap in between two of the teeth and in side corner of teeth it's getting black strain developed and now in upper incisors teeth having black strain is developing.As you can check in the attAched image kindly suggest me what should I do and how to stop or remove that black strain. Thanks in advance.

Hi Dr I am having cavities from school days I don't have any pain or something I have 8 cavities all molar teeths below 4 teeths and exact same position 4 teeths above although it appears to be small green spot in the middle of the teeth I worried could it cause any pain in future years..
