From the given clinical photograph it seems that, there child has deep caries with molar tooth of upper jaw which is causing pain .

Seeing the image the age of the patient may be 8 to 10 yrs..it looks like two milk teeth got damaged badly. The pain may be in  one or both the primary teeth. The treatment  plan can be decided after clinical examination and x ray examination and condition of the tooth. For immediate pain relief u can use a tab.ibugesic Jr or tab.paracetomol- 500 mg ..twice daily for not more than 3 days

This tooth seems to have a big cavity, as seen in your picture. The child must be in real pain. For the time being you can give him a painkiller tab, but he needs a proper dental treatment from a Pedodontist.
