Tooth and Mouth Problems

I've been facing mouth sores problem frequently. I need a permanent solution for this issue. Please help.

Headache a lot in everytime and a lot of pain in the upper right jaw and looks like a fever also. i went to dentist before 5 or 6 days he made a cap for my lower jaw and at that time he set my upper jaw like that but my upper jaw is panning a lot what should i do.....?

I am 18 years old and was in a habit of smoking since last three months but now I've stopped after noticing the fact that I am constantly having pain in the right lower jaw, also I've noticed some red lesions on the behind lining of the tongue. Just few days before noticing that I am also having difficulty in opening mouth and swallowing anything!

It's bleeding since last 2-3 months as well as I am feeling that bond between teeth & gums as well as smell is also there?? Actually my father faced same problem in age of 27-28 and lost all his teeth in age of around 30. I don't want to be like that so please suggest me??
