Tooth and Mouth Problems

I am 18 years old and was in a habit of smoking since last three months but now I've stopped after noticing the fact that I am constantly having pain in the right lower jaw, also I've noticed some red lesions on the behind lining of the tongue. Just few days before noticing that I am also having difficulty in opening mouth and swallowing anything!

It's bleeding since last 2-3 months as well as I am feeling that bond between teeth & gums as well as smell is also there?? Actually my father faced same problem in age of 27-28 and lost all his teeth in age of around 30. I don't want to be like that so please suggest me??

The gums on the inside of the mouth especially the back ones (gums of molar and pre molar) are paining very badly. I am not able chew anything. The moment I chew I fell like I am trying to eat pointed stones. Any help will be appreciated.
