Tooth and Mouth Problems

My one teeth problem and one doctor suggested teeth removed but I am don't trust this doctor because doctor prescription no show any degree... Please help me

I m chewing tobacco for 5 years ....2 months before i saw black patches inside my lips where i put tobacco.....so i checked with dentist and she told me that it is reversible if i stop using tobacco....but i didn't.....now they looks more black and also i stop putting tobacco in one side and that side spots are disappeared....is this melonosis? Is this serious issue? Please suggest me.

I think I need need dental fillings done due to cavity and large pits in my teeth .is it painful procedure? Do dentist give injections for it ? I am very nervous about the procedure.
