Hello I'm having severe tooth pain from last few hours . What do I have to do please suggest me doctor

I am undegoing through rct of my tooth but pain doesn't Stop and doctor has asked me to take up next step i. e. Surgery

I have undergone through rut followed by caping one year ago now when I press gum under the cap small blood has appeared at the line between cap and gum.

I have started recently my orthodontist treatment.but while extraction of teeth I m going sudden unconsciousness when doctor give me injection in my mouth.totally unconsciousness . before extraction I haven't take any breakfast or meal..Not single peice of food..Just taking single cup of tea I went for tooth extraction..Plj What r the reason for my sudden unconsciousness ??? Is their anything serious ??
