Experiencing irritation or itching sensation in the rectal region while passing stool. There is no blood, only irritating sensation. What to do?

I'm unable to excrete properly , I feel like full always , even my stomach feels like a bit swollen and I always put out in small amount but I fell like stomach full .....

Having stomach pain and frequently going to toilet from 3 days , having burning sensation in anus once after the toilet.

I have been having sugar,fatty liver grade2,gall bladder stone,thyroid Tsh 7.9,cholesterol

I have been detected with stomach ulcer. kindly give medicine, how many times to take those medicines and for how many days. I'm male, 38 yrs old.

My grandmother 73 year old is suffering from Constipation: Stool type 1 from last 8 to 12 months. Doctor has advised Duphalac syrup but it is not helping much. Is there anyway to treat this problem??? She is struggling a lot and even after using Duphalac syrup 5 ml a day, she finds it really tough. Stool type 1 or sometimes it doesn't come out for 2 to 3 days. Any medical help/ advise would be greatly appreciated. Thank you.
