Heart burn, chest pain and sometimes headache and dizziness,feeling nausea,  Please check the report

I am having chestburn continuously for more than six month, i eat rich food fried food i kniw this causes acidity, i used eno initially but now its scaring me as now the burning sensation is continuos throughout the whole day, i quitted smoking but still.. do i need to worry something seriously or it is a problem but not scary. Kindly clear my doubts and explain in detail. What to do n what not to . Thanks and regards
