Skin, Hair and Nail Problems

Hi dr! . I started to have itchy scalp mainly at the cowlick area and backside of hair, when I touch I sometimes feel bump in hair and when I scratch it with my nail I feel some flakes in moving I don't know how to tell ,it's been a month .i have appointment with Dr tomorrow .i want to know what's happening

My frontal hair is full but why vertex hair scalp is visible is this hair loss ? If yes then what stage is this? I'm just 24

Been feeling slight pain in toes and looked and saw this dry, flaky thing between pinky toe and ring toe. There also looks like there's holes. I have been walking in tight shoes for the past 2 weeks all day due to some work could it be due to that or is it another issue. I am pre diabetic only. No other health issues.

I had itching on my genital area and it is spreading towards my stomach, it looks like patches causing itching, now I put powder on it but it's not working, itch gaurd and ring gaurd gives me temporary relief but again it is coming back after one week once I applied cream. It's been since long back around 10 yrs since it is continuing, plz tell me What to do. Once I had fluconazole 150 mg fo two weeks after that it was ok for next 2-3 weeks but it started again thereafter. This is happening again and again on and off . Now i want permanent solution for it as this itching is causing me very much.
