My father having diabetes and hypertension and recently found creatanin level is 2.15 Now what to do

I have delay in start of peeing,and some times feel pain in stomach,,,,,,and lower back ...What would the problem

Pateint is skin allergic, is diabetic since last ten years, has body swelling, pain in hip area, treated with hyponitremia recently.

Got a complete set of tests (CBC,Thyroid, Sugar, Urine) and showed 20-25 pus cells. I was having severe lethargy but no fever or abnormal symptoms. Sugar & thyroid report was clear & no major abnormality in CBC. Urine culture was clear. GP asked to take Cital syrup but still feel very sleepy during the day. Been a week but not feeling fully fit.
