Today my went to cardiologist for routine cardiac check up where he advised him echocardiography wherein the results are LVEF 58% Concentric LVH and grade 1 diastolic dysfunction.

Mild chest discomfort
I woke up with mild chest discomfort, not exactly pain, but to describe it, its like a feeling of some ant travelling in a nerve, this was temporary and felt it 5-6 times intermittently, sometimes while taking long breath.
On walking, I felt mild pain in my left shoulder and back.
My cholesterol is normal but HBA1C is 6.4 (6 months ago it was 5.8, I had my wedding and food habits got worsened and substantial weighed gained)
Height 5'5' Weight 73.5 kg
I got my ECG and Echo done, both were normal.
The sensation and left side pain I can feel intermittently for a week since that incident. I am worried that is it angina?

Mitochondrial diaease and heart failure
I am having symptoms of heart failure like extreme shortness of breath, pulmonary congestion, rapid and irregular heart beats, low urine output with brownish urine, sometimes chest pain.
Changes in ECG(Significant ST-T changes)
Doctors have evaluated me for traditional heart failure. They did 2D echo(LVEF 65%), Cardiac MRI(LVEF 66%), Coronary Angiography(No Evidence of CAD), Nt Pro BNP (60 pg/ml). Now doctors are suspecting for cardiac metabolic disease like mitochondrial disease.
Doctors have told me if it is there, there is no treatment for it and it can cause severe heart failure and death. How to treat this condition? What is the diagnostic test for this disease?

Hypertension
I was on Dilzem CD 120 capsule twice a day for hypertension since 4 years.
The reason for taking Dilzem is that due to a neuro medicine tryptomer which I took for headache my heart rate went up.
Also at that time bp gone high. So cardiologist gave Dilzem CD 120 .
Recently BP gone more high to 150/100 and sometimes diastolic pressure went till 110.
So telma 20 was added.
Then average bp became 130/90.
Sometimes 120/86 .
So cardiologist advised to stop one Dilzem and try.
But again it became 140/100.
So now should I go back to Dilzem 120 twice a day and telma 20 once a day?
Or should I take telma 20 twice a day and Dilzem once a day?
I got different suggestions from different doctors.
Hoping to get suggestions.
