I hv been suffering from cough within 2 weeks and whenever i cough i get pain in my chest please tell me wt should i ll do

I have been facing pulses type pain on left side of chest continuously for 5-6 days. Sometimes in middle too. And left hand pain also link to this left side part.

Being smoker 8-10 cig a day & alcoholic my bp is all time high like 150/100 , I swear also @ temperature higher than 30'C. I don't do exercise, I eat veg but very spicy food being kolhapur.

I started following keto diet. Since April. Ever since my cholestrol has been going up. Though triglycerides are in normal range. I am 38 years. Active person. I am concerned that do i need to start medication or shld i stop keto and follow normal low carb diet to get heakth on track.?
