Persistent chest discomfort.left arm and shoulder pain dizziness fainting short of breath tirdness

Cholesterol is 215 Hdl is 33 Ldl is 113 Triglyceride is 259 Plz suggest what should I do immediately. Do I need medication or just life style change. I am male 33 years weight 70 kg height 5.7

My father has first heart attack in march 2012,then 3 heart blocks are found.no strain or bypass r not completed.then 2017 2nd angiogram17 march was done.2 strain was needed.but i wanna go to india for better treatment.
